Modernizing national safety operations using advanced tech

The Veterans Health Administration (VHA) is the nation’s largest health care system, with 1,380 facilities serving 9.1 million Veterans each year. VHA’s National Center for Patient Safety (NCPS) leads efforts to prevent harm, identify risks quickly, and share safety information.

Previously, NCPS used two outdated systems that were hard to use, had limited reporting, and made communication difficult. Important information often got lost in emails or spreadsheets, slowing down responses and making it hard to manage safety actions. When critical safety information is delayed or missed, Veterans are put at real risk of preventable harm, from exposure to recalled medical products to delays in addressing emerging clinical safety issues, where every moment can directly impact patient outcomes.

Solution: Engineering Two Production Platforms From the Ground Up With Salesforce

Booz Allen architected and engineered two enterprise Salesforce platforms from the ground up, enabling real-time data visibility, workflow automation, and system-wide safety management.

Alerts and Recalls (A&R)

Booz Allen engineered A&R to automate urgent safety alerts across 1,380 facilities for real-time safety alerts and recall instructions so facilities can act fast. The system is used by 6,138 users in 28 roles. Facilities update their records, and national leaders see actions in real time. What used to take hours now happens in minutes. In 2025, 1,034 safety records were created and 216,064 action assignments completed.

Patient Safety Assessment Tool (PSAT)

Booz Allen developed a proactive safety assessment platform that handles large safety surveys and helps facilities spot hazards early. Six hundred and ninety-five users in 15 roles use it for checks, compliance, and action plans. In 2025, Booz Allen designed the survey framework to support rapid deployment of assessments while allowing flexibility to support diverse clinical environments—560 new surveys were published, with the question bank growing by 212 new questions, a 748% increase over 2024, allowing PSAT users to mitigate 1,046 risks on time, up 44% from 2024.

As one Booz Allen engineer said, “Salesforce let us combine national safety rules with local realities, without making everyone work the same way.” The new systems centralize workflows, reporting, and communication. They support both national standards and local needs, making it flexible and efficient. Users get tailored workflows and reports, improving teamwork and speed.

Both applications offer strong analytics and visibility. The national team can track participation and risk reduction in real time. Leaders can spot delays and address issues instantly. For PSAT, there are 3 dashboards and 95 reports; for A&R, 22 dashboards and 178 reports. Data is sent nightly to the VA’s Summit Data Platform (SDP), so analysts always have up-to-date information.

Impact: Safer Care for Veterans

This work was recognized with a FORUM Innovation Award, setting a new standard for innovation in patient safety for Veterans. With the platforms Booz Allen engineered, communication that used to take hours now takes minutes; national leaders gain real-time oversight, turning reporting into active management; and facilities now have systems for actions, approvals, and extensions, improving clarity and consistency. Most importantly, these changes mean safer care for Veterans.

The VA project manager rated the new applications at the highest level in a customer satisfaction survey, showing trust and satisfaction with the solutions. Users value quick support, clear communication, and how the platform keeps everyone aligned and working efficiently. These improvements help not only Veterans but also their families, giving everyone peace of mind.

By designing, building, and scaling these modern platforms, Booz Allen transformed how NCPS operates, enabling nationwide coordination, continuous monitoring, and rapid response. Veterans and VA staff now experience a fundamentally different system, one where critical safety information is delivered in real time, risks are identified and addressed proactively, and teams across the country are aligned and able to act with speed and confidence. Clinicians and staff no longer rely on fragmented communication or delayed reporting; instead, they operate with clear visibility, streamlined workflows, and immediate access to the information they need to protect patient safety.
