Kelly’s point around the need for coordinated care at a population level is key for the public health impact of health reform. For the real impact from some of the key initiatives including HHS’ overall Prevention and Wellness programs, having information from across the system and being able to consolidate the data in a meaningful way for analysis across a population is key to divining the need for interventions (e.g., which populations, communities, etc) and then evaluating the impacts of those interventions. Again, having that coordinated care model where that spectrum of care data is documented and gathered is essential for the overall advancement of public health initiatives particularly around chronic disease prevention and overall wellness.
